Individual Health Care Plan

An individual healthcare plans helps us to ensure that we can effectively support your child with any medical conditions. They provide clarity about what needs to be done, when and by whom. It is essential, such as in cases where conditions fluctuate or where there is a high risk that emergency intervention will be needed (i.e serious allergies, diabetes, asthma). It is also helpful in a majority of other cases, especially where medical conditions are long-term and complex.

With an Individual Health Care Plan students at Christ the King with medical conditions are properly supported so that they have full access to education, including school trips and physical education. We consult health & social care professionals, students and parents to ensure that the needs of those children are effectively supported.

The Headteacher is the named contact in school who has overall responsibility for ensuring students with medical conditions are properly supported. She is responsible for ensuring that sufficient staff are suitably trained and a commitment that all relevant staff will be made aware of the child’s condition, She is also responsible for risk assessments for school visits and other school activities outside of the normal timetable.

These forms can be downloaded, filled out digitally and emailed to office@christtheking.notts.sch.uk, or printed and filled in by hand and handed in to Student Services. Please complete a new medication form if your child’s medication changes.
