Sixth Form Application Form
We expect our students to complete a sixth form application form which outlines the three subjects that they wish to study. We also ask for applicants to enter a reserve subject as it is not always possible to gain the three subjects that they wish to study. Also located in the application form is a section where applicants need to outline why they would want to attend our sixth form.

Once you have completed the form, please return to Mr Reddington at nreddington@christtheking.notts.sch.uk or pass to your form tutor if you are a Christ the King Year 11 student.
Home School Agreement
Each student and parent is also expected to sign the Home-School agreement which is contract outlining the expectations of the school, the student and parents in supporting their child to be successful during the Post-16 education.
